Transportation Coordination – Uber Health
Everlasting Care Home Health provides transportation coordination for eligible clients through Uber Health. Our team schedules and monitors rides to help clients safely travel to appointments, services, and community activities. This support is especially helpful for individuals receiving Medicaid waiver services or those who need assistance accessing care.
Our caregivers assist with activities of daily living, including bathing, dressing, and grooming, helping seniors maintain their independence and dignity.
